Available resources and services from the Cudahy Health Department and other agencies.

Health Insurance Assistance

Milwaukee Enrollment Services (MilES) staff members are available to assist with enrollment in BadgerCare Plus for Families and FoodShare as well as answer case questions and assist with reviews.

MilES staff members are always available to answer your questions over the phone at 1-888-947-6583 between 8:00 a.m. and 4:30 p.m. weekdays.  Applications for services are also available online at www.access.wisconsin.gov.   

Resource Library

The Cudahy Health Department has a large selection of pamphlets available to residents upon request.  Topics include infant/child health issues, behavior, growth and development as well as information about adult health issues.  Contact the Health Department for more information.

Hospital Equipment Lending Service

  • Short-term loan of hospital equipment is available to any Cudahy or St. Francis resident through the Cudahy-St. Francis Kiwanis Club.
  • A variety of equipment, including wheelchairs, walkers, commodes and bath chairs, are housed at the Cudahy Health Department. 
  • Call the Cudahy Health Department during normal business hours to be sure that the equipment you need is currently available.
  • Equipment is normally lent out for 60 days and can be renewed for an additional 60 days.

  • Badger Care Plus (For Income Eligible Pregnant Women, Children, and Families)
  • Badger Care Core Plan (For Income Eligible Adults Age 19-64 with No Dependent Children)
  • Food Share  (For Income Eligible Individuals or Families)

Appointments are available by appointment on the first and third Monday of each month between 9 a.m. and 3:30 p.m.

  • Who can apply for BadgerCare Plus?
    All children under 19 years old at all income levels who do not have access to health insurance.
  • Pregnant women (up to 300% of the Federal Poverty Level, which is $52,800 for a family of three).
  • Parents and caretakers at higher income levels (up to 200% of the Federal Poverty Level, which is $35,200 for a family of three).
  • Young adults who are leaving foster care when they turn 18 (regardless of income).
  • Parents with incomes up to 200% of the Federal Poverty Level who have children in foster care.

FoodShare

Individuals or families with an annual income at or below 130% of the federal poverty level ($14,079 for an individual, $23,803 for a family of three) may be eligible.

WIC

WIC is a supplemental nutrition program which is provided by the Cudahy WIC Program.  The Health Department holds clinics on Wednesdays, Thursdays and Fridays from 8:30 a.m. until 4:00 p.m.   Pregnant and breastfeeding women, new moms and children under the age of five years can receive vouchers for infant formula and other types of healthy foods, as well as nutrition education and health screening. There are no residency requirements.

Wisconsin Well Woman Program

WWWP provides free mammograms and/or pap tests for low-income Cudahy women 50 yrs of age or older.  Call the Cudahy Health Department at 769-2239 ext. 302 for more information.

Family Planning Waiver Program

Are you a woman between the ages of 15-44 wanting birth control services?  Do you need insurance for birth control services?

Income-eligible women seeking confidential birth control services can receive this health care through the Family Planning Waiver Program.  For more information, call the Cudahy Health Department at 769-2239 Ext 303.

Free Medical Care

The Packard Avenue Community Clinic at 5854 South Packard Avenue (across from Pick 'N Save) offers free medical care for people with NO HEALTH INSURANCE.  The Clinic is open every Tuesday from 6:00 – 8:30 PM, on a first come, first serve basis.

Services include:

1. Short-term basic health care from a primary care physician.
2. Women's health (No pregnancy testing, birth control or STD testing)
3. Health screening and Risk assessment
4. Education and counseling
5. Referral to appropriate community service
6. Podiatry referrals

This clinic is sponsored by Lakeshore Medical Clinic with support from St. Luke's South Shore.
